Department of Energy Heating Assistance Program Application Dates
The Department of Energy of the Lumbee Tribe of North Carolina is pleased to announce the 2015 Heating Assistance Program (HAP) application dates. To be eligible for assistance all applicants must meet the income requirements set by the United States Department of Health and Human Services.
APPLICATIONS WILL BE TAKEN ON THE FOLLOWING DATES: (All sites will be closed on November 11 in Observance of Veterans Day)
October 5 – 23, 2015: Applicant who is a Veteran or Elder 65 years of age or older
October 26 – 30, 2015: Applicant receiving disability assistance or household with children 5 years of age or younger
November 2 – 13, 2015: Any applicant who may be eligible for assistance
APPLICATIONS WILL BE TAKEN AT THE FOLLOWING SITES:
Department of Energy, 6984 Highway 711, Pembroke, NC (Robeson): Monday – Friday between the hours of 9am – 3pm
Lumbee Heritage Elders Court, 16220 Lumbee Heritage Lane, Laurinburg, NC (Scotland): Tuesday between the hours of 9am – 3pm
Eagles Nest Apartment, 2200 Indian Creek Road, Fayetteville, NC (Cumberland): Wednesday between the hours of 9am – 3pm
Hawkeye Boys and Girls Club, 3066 Blue Springs Road, Red Springs, NC (Hoke): Thursday between the hours of 9am – 3pm
THE FOLLOWING INFORMATION IS NEEDED TO COMPLETE AN APPLICATION:
• Picture ID for Head of Household • Social Security Card for Entire Household or Tax Document • Birth Certificate for Entire Household (Applicant may also use tribal enrollment card, Medicaid card if birth certificate cannot be provided) • Verification that an American Indian Resides in the Household • Monthly Household Income including Annual SSI/SSA Statement • Monthly Checking and/or Savings Account Bank Statement • Bill/Monthly Statement for heating cost (If Applicable)
